Get Connected To Primary Care In Northern & South Bruce Peninsula

As part of the provincial initiative for all Ontarians to be connected to a Primary Care Practitioner by 2029, the Peninsula FHT & the South Bruce Peninsula FHO want to ensure that all residents have updated information on how to find a local Physician or Nurse Practitioner if they are currently without care.

If you are a local resident who is currently without care, you should register with Health Care Connect to get added to the waitlist.

Health Care Connect is a provincial initiative that will help Ontarians to find a family physician, nurse practitioner or primary care team who are accepting new patients in our communities.

Patients can easily register for the program:

• by calling 811, available 24/7

• online at ontario.ca/healthcareconnect

To successfully register, patients must have:

• a valid OHIP card

• an up-to-date mailing address

• complete a basic health questionnaire 

The Health Care Connect team will work with local clinics to help connect residents with a primary care provider in our area as soon as possible. Priority is given to individuals who have greater health needs. We encourage all residents who are currently without care to add their name to the Health Care Connect waitlist as it will provide our clinics with updated information on the number of residents in need of care in Northern & South Bruce Peninsula.
